Health & Wellness Coach in Training Application Form

Live Disease Free seeks a certified or experienced health and wellness coach to join our team.

This coaching position begins with 3-6 months immersive training in student support, where you'll master our proven system while supporting students via email. You'll then advance to direct coaching responsibilities.

We're looking for a coach who brings expertise AND humility—confident in their coaching foundation but eager to learn our specific approach.

Section 2: Health Coaching Background

We're hiring a health coach to learn our system. You must bring foundational coaching knowledge—we'll train you in the Live Disease Free methodology.

Describe your coaching experience in detail:

  • What is your coaching philosophy/approach?

  • What results have you helped clients achieve?

Section 3: Core Values Alignment

Our team operates on four foundational values: Compassion (caring & kindness), Integrity (honesty & doing what's right), Humility (teachable, servant-hearted), and Faith (trust in God; hope-centered).

Section 4: Written Communication Assessment

Since this position begins with student email support, we must assess your written communication skills. Please draft replies (150–200 words each) to these mock student emails.

Guidelines:

  • Warm, encouraging, professional

  • Make students feel heard and supported

  • NEVER provide medical advice

  • Demonstrate appropriate boundaries

  • Show compassion, integrity, and hope

  • Excellent grammar and spelling required

Section 5: Video Introduction

Record a 2-3 minute video introducing yourself and answering:

  1. What are your strengths in coaching and student support?

  2. Why are you a good fit for this role and our mission?

  3. Why are you interested in becoming a Live Disease Free Wellness Coach?

Presentation Tips:

  • Business casual attire

  • Quiet space, good lighting, minimal background distractions

  • Speak clearly, maintain eye contact with camera

  • Be authentic; let your compassion show

  • Stay within 2-3 minutes
